Each form contains the name and personal information of the applicant, including their profession and length of work, his the name of his sponsor, and the signatures of the approving committee members. In addition, in printed text, each form includes the terms and conditions associated with making an application to the brotherhood.
Contains a list of handwritten entries, identifying beneficiaries in the event of death of members of union, organized alphabetically. Each entry includes the name of the member, the beneficiary, the relationship between the two, membership information, and the date and number of the relevant beneficiary certificate.
